A sample form is offered.  The first part is shown below to give an idea of how it works.

The form may be amended and adapted by a Local Meeting to suit its own needs.  We are suggesting items that might be present, or might need adding, you may well think of others that are particular to your situation, consult with Friends in your meeting, be creative, seek guidance from any local organisations of disabled people, or your Council Access Officer (if there is one).
